UC Irvine Freshman Contract for
Provisional Admission 2009-2010
Please read this contract before submitting your Statement of Intent to
Register (SIR) and be sure to keep it for your records. Submitting a
positive SIR and accepting this offer of admission certifies that you
understand and agree to comply with all of the conditions of this provisional
contract.
Provisions for Admission to UC Irvine
1. You must:
- Receive a high school diploma or equivalency.
- Complete all senior-year courses as listed on your application.
- Maintain at least a weighted 3.0 senior-year GPA in UC-approved
courses.
- Notify the UCI Admissions Office immediately if you receive a D or F
grade in any A-G or college course in your senior year. Use the Message
Center on MyAdmissionsApplication@UCI at www.admissions.uci.edu.
Provide your grade report, submit all high school official transcripts, and
then consult with your Admissions Counselor. Be advised that your admission
will be subject to cancellation if your official transcript includes D or F
grades in your senior year, or D or F grades in other semesters unreported
on your UC Application.
- Notify the UCI Admissions Office using the Message Center if there are
any changes to the A-G courses reported on your UC Application for
your senior year. Your admission will be subject to cancellation if you fail
to maintain the level of academic challenge in the coursework presented at
the time you were offered admission.
2. By July 15, 2009, you must:
- Submit an official final high school transcript to the UCI
Admissions Office that confirms the exact date you graduated (month, day and
year) and verifies your self-reported academic record. Request this transcript
well before the end of your senior year.
- Submit official test scores for the correct required
standardized tests, if you have not already done so. Official
standardized tests required by the university include: (1) the ACT Assessment
plus Writing, or the SAT Reasoning Test, and (2) two required SAT Subject
Tests in two different subject areas: English, history and social studies,
mathematics (Level 2 only), science, or language other than English. To
request that scores be sent to UCI, contact the Educational Testing Service
(ETS) at (609) 771-7300 or (888) 225-5427. Our ETS code is 004859.
- Submit an official report for all of your Advanced Placement
(AP) examination scores (3 or above) and/or International Baccalaureate
Higher Level (IB) examination results (5 or above) taken prior to
January 1, 2009. To have AP scores sent to us, call the Educational Testing
Service at (609) 771-7300 or (888) 225-5427; our ETS code is 004859. Please
note: to ensure we receive a complete set of AP scores, you need to
individually specify the test year(s) that the AP Exam(s) were taken when
you make your request. Contact the International Baccalaureate Office at
(212) 696-4464. Official reports are those sent directly to us electronically
by these services.
For exams taken after January 1, 2009, submit an official report for all of
your AP scores (3 or above) and/or IB scores (5 or above) as soon as the
results are available or they may not be credited towards your record.
- Submit an official transcript of all completed college/university
work. If the UCI Admissions Office does not receive the above required
information by July 15, a "hold" may be placed on your records; if left
unresolved, this will lead to enrollment cancellation from any courses for
which you registered during open enrollment. The UCI Admissions Office is not
responsible for enrollment cancellation from courses, or admissions
withdrawal decisions after the start of the fall 2009 quarter if you submit
official transcripts, test scores, or other information after the July 15
deadline.
Your admission to UC Irvine will be
cancelled if you do not satisfy all of the above conditions or
any information you submitted does not agree with official documentation,
including failing to report all schools attended, or all A-G courses completed
(original failing grades and grades for courses repeated). You are solely
responsible for the accuracy and completeness of all information reported as
detailed in Section VII and Section XIII, item 137, of the University of
California undergraduate application for admission and scholarships.
If UCI's offer of admission is withdrawn, the Statement of Intent to Register
fee is nonrefundable, and the UCI applicant is responsible for contacting
UCI offices to arrange for any reimbursements due for housing, registration,
financial or other services.
