Health

The Student Health Center will send you medical history and physical exam forms to be completed and returned prior to enrolling at UCI. Students under 19 years of age when they enter UCI are required by State law to provide proof of full immunization/immunity against the hepatitis B virus. These forms are also available for download. Go to www.shs.uci.edu and select "Admissions Health Requirements."

HEALTH INSURANCE coverage is a non-academic condition of enrollment for all undergraduate students. One insurance plan available to students is USHIP, UCI's Undergraduate Student Health Insurance Plan. Its premium is already included as part of your student fees. If you have comparable, verifiable insurance coverage, you will be able to waive the health insurance fee by completing a waiver form on the Web. Information detailing insurance benefits and instructions on how to do that will be mailed to all students prior to the fee payment deadline. You may also read about this on the Student Health Center website. Go to www.shs.uci.edu and select "USHIP Waiver."

Disability Services

NEW UCI STUDENTS WHO HAVE PERMANENT DISABILITIES and who want to register with and/or request disability accommodations from the Disability Services Center (DSC) must submit professional documentation. Adequate documentation must be received by DSC before any accommodation request can be considered. "Permanent disability" encompasses physical, learning, and psychological disabilities; being legally blind or partially sighted; being deaf or hard-of-hearing; or having Attention Deficit Disorder.

NEW STUDENTS WITH DISABILITIES MAY QUALIFY FOR PRIORITY REGISTRATION on WebReg. To qualify for this, DSC must receive adequate disability documentation by May 21. Disabled students who need accommodations are urged to submit adequate documentation at least three weeks before the start of classes (September 21). Failure to do so may delay or preclude provision of certain accommodations for the quarter. Call or visit DSC for information about documentation requirements, services, and accommodations.
